These terms govern your use of this website, operated by OakClaim ("OakClaim", "we", "us"). By using the site, you agree to them. Services we perform for billing clients are governed by a separate written services agreement, which controls if it conflicts with these terms.

Reference tools and content

The site includes free reference tools — including the BCBS prefix directory and the denial code library — and educational content about medical billing. These are provided for general informational purposes only:

  • No guarantee of accuracy.Payer information changes constantly and without notice. BCBS alpha prefix assignments are not published by any authoritative public source, and our directory is compiled from publicly available information. Always verify with the payer, the member's current ID card, or official channels before filing claims or making billing decisions.
  • Not professional advice. Nothing on this site is medical, legal, coding, or compliance advice, and it does not create a client relationship. Consult qualified professionals for advice on specific situations.
  • Claim adjustment reason codes are maintained by X12; our descriptions and guidance are original editorial content.

Acceptable use

  • Do not scrape, bulk-download, or republish the site's datasets or content without our written permission.
  • Do not use the site to transmit malware, probe or disrupt our infrastructure, or attempt to access systems or data not intended for you.
  • Do not submit false, misleading, or unlawful information through our forms.
