How BCBS alpha prefixes work
Blue Cross Blue Shield is a federation of independent local plans. The three-character alpha prefix at the start of a member ID tells you which plan "owns" the member — which determines where eligibility is verified, where the claim is routed through the BlueCard® program, and whose timely filing rules apply. Getting the prefix right up front prevents the most common BCBS denials: wrong payer, eligibility mismatches, and expired filing windows.
